Abstract
Objective: To assess the current status and identify factors associated with the quality of nursing documentation in medical records in accordance with Circular No. 32/2023/TT-BYT at Ha Dong General Hospital in 2025.
Methods: A cross-sectional descriptive study was conducted on 320 inpatient medical records from four surgical departments between January and July 2025. Data were collected using a validated instrument and analyzed with descriptive statistics and measures of association (odds ratios [ORs], 95% confidence intervals [CIs], p < 0.05).
Results: Overall, 88.8% of medical records met quality criteria. By component, nursing care and monitoring records achieved 82.5%, inpatient service disclosure forms 93.8%, and infusion records 95.3%. Factors significantly associated with documentation quality included age group, educational level, and participation in training.
Conclusions: The overall quality of nursing documentation at Ha Dong General Hospital was good. Training and supervision should be strengthened, with priority given to nurses younger than 30 years, those with educational levels below a bachelor’s degree, and those who have not been trained on Circular No. 32/2023/TT-BYT.
